It was working at 97 percent capacity when the accident occurred around 4 a.m. on the morning of March 28, 1979. A slight issue in the secondary cooling circuit caused an increase in the temperature of the primary coolant.
In addition, the primary coolant had been depleted to the point that the reactor core’s remaining decay heat could not be dispersed. As a result, severe harm was done to the body.
Because of this, the reactor’s operators had no idea what had caused the reactor to shut down in the first place. A lack of emergency response training and a lack of control room equipment was cited as the primary causes of the accident, according to the report.
